Passport Information

Items required

  • Completed application – Applications take about 20 minutes to complete
  • Evidence of Citizenship –
    • certified copy of birth certificate (hospital birth certificates are not acceptable), or
    • previous passport, or
    •  naturalization papers
      • all evidence of citizenship documents are mailed with the application, but will be returned after the passport is created  
  • Social Security #
  • 2 colored passport photos -- (taken within the last six months) 2x2 inches white background
  • Proof of identity – valid driver license, government, military, or state ID, or previous passport
  • Applications for children under 16 require the signature of both parents. If the second parent is not available to sign, the appearing parent must:
    Present evidence that he/she has
    • sole legal custody of the child, or
    • notarized written consent of the other parent for the issuance of the passport, or
    • written statement explaining why non-applying parent's consent cannot be obtained.
  • All minors must appear in person when applying for a passport
  • If a child under 16 has a previous passport a birth certificate is still mandatory

    Fees

    All fees are regulated by the U.S. Department of State, Passport Agency.
                (Effective July 13, 2010)

    • Each passport book and card has a $25.00 acceptance fee in addition to the following fees – payable to the Davis County Clerk/Auditor
    • The following fees are payable to the U.S. Department of State

    (The US Department of State fee is mailed with the application and must be in the form of a check or money order for each individual application.  If a check or money order is not available, a debit or credit card may be used for a $2.00 fee per application.)

    PASSPORT BOOK
    (valid for all land, air and sea travel)

    Applicants 16 and older

    • $110.00 for regular processing (6-8 weeks)
    • $170.00 for expedited processing (3-4 weeks)
    • $170.00 plus current overnight mail charges (7-10 business days)

    Applicants 15 and younger

    • $80.00 for regular processing (6-8 weeks)
    • $140.00 for expedited processing (3-4 weeks)
    • $140.00 plus current overnight mail charges (7-10 business days)
    PASSPORT CARD
    (only valid for land border crossings or sea ports-of-entry from Canada, Mexico, Caribbean and Bermuda)   

    Applicants 16 and older

    • $30.00  for regular processing (6-8 weeks)
    • $90.00 for expedited processing (3-4 weeks)

    Applicants 15 and younger

    • $15.00 for regular processing (6-8 weeks)
    • $75.00 for expedited processing (3-4 weeks)
